We help execute life planning process that will support our client’s desire to achieve life goals. PCP will help build based on the client’s strengths to engage in activities in their community and will honor our client’s preferences, choices, and abilities.
General Self-Directed Supports (099)
General SD supports occurs after a potential participant completes a Self-Determination Program (SDP) orientation and before enrollment in the SDP. Provide assistaning and coaching and/or training individual
Independent Facilitator (340)
Assisting participants and families in Self-Determination Program (SDP) once have entered, you can choose Ongoing Independent Facilitation Services. Services include tracking your spending plans, provide checklist for steps to stay on track during the Self Determination process, coordinating with Financial Management Services (FMS), updating spending plan, classifying and executing generic resources first, assisting with staff supervision, assisting with negotiations and pricing with vendors.
